I am requesting that the Office of Information Technology (OIT) make a "best faith effort" to troubleshoot and repair my computer. I confirm that all of my data has been backed-up and that I have access to it from another source, such as Google Drive or a portable hard drive.
I acknowledge that OIT will not be held responsible for the loss of any data/files that may be irretrievable due to deletion or media damage. OIT efforts to back-up/restore, or recover information from my hard drive is not a guarantee that your data will be retrieved.
If this is a personally-owned computer, there is a minimum fee of $20 (Bench Fee) for OIT to diagnosis the issue and if possible offer a solution.
If they do offer a solution to the problem, and you agree to have the work done, the $20 Bench Fee will be waived and all work is subject to $40/hour plus the cost of any parts.
NOTE: This service may take up to 10-business days (2 weeks) to complete.
